Seeking Home Health Nurse Practitioner for a locums contract in Michigan:

• Start: ASAP

• Schedule: 5 days per week
  • Preference is M-F, but willing to accommodate alternate schedules
• Setting: In home visits to health plan members
• Volume: 5-6 patients/day
• Location: Will start in Kent County, MI but will be reassigned throughout the state as local volumes are cleared
• Licensure & Credentials:
  • Must have an active MI NP & RN
  • Certification : FNP-C or GNP, must be board certified in Family Practice, Internal Medicine, or Geriatrics
  • Payer credentials : Medicare & Medicaid enrollment required
• Charting: Athena

Provided/Reimbursed: Travel (flight to assignment, flight back home), hotel, medical equipment (not clothing), rental car/gas or mileage reimbursed
